Personal Data Protection Law Application Form

1. DATA SUBJECT’S RIGHT TO SUBMIT A REQUEST

Under Article 11 of the Personal Data Protection Law No. 6698, certain rights have been granted to data subjects. Applications regarding these rights must be submitted to us in accordance with Article 13 of the Personal Data Protection Law, by completing the following information and sending it to our company in writing via registered mail with return receipt.

The data subject must fill in the following information and attach identity verification documents, either in person or by post, to the address “ATILGAN AKVARYUM İNŞ.SAN.TİC.A.Ş.KAŞ PENİNSULA GARDENS BRANCH, Selahattin Sönmez Cad. Andifli Sok. No:6 Cukurbağ Peninsula Kas / Antalya” or by email to [email protected] kep address, or through a notary public or by using the email address previously notified by the relevant person and registered in our company’s system to [email protected] address. In order for the personal data owner’s application to be responded to more quickly, we request that ‘Personal Data Information Request’ be written on the application envelope.

We will respond to applications made to us in writing or electronically within 30 days at the latest from the date your request reaches us.

2. Application Owner Requests

For the items you wish to apply for, you can indicate your requests by placing an ‘X’ in the ‘Your Preference’ column of the table below.

1. Are my personal data processed by your company?
2. If my personal data are processed by your company, I request information about the purpose of the processing and whether the processing is carried out in accordance with the purpose of the activity.
3. I request information about whether my personal data has been transferred to third parties within or outside the country and, if so, with whom it has been shared.
4. Considering that the reasons for the processing of my personal data no longer exist, I request the destruction of this data by selecting an appropriate method (deletion, destruction, anonymisation).
5. I believe that my personal data has been processed incorrectly or incompletely. If you believe that any of the information/documents requested in this clause is incorrect and should be corrected, please provide us with the correct and complete information/documents.
6. If the reasons for processing my personal data no longer exist, I request that this situation be communicated to any third parties to whom my data has been transferred.
7. I request compensation for any damage I have suffered as a result of the unlawful processing of my personal data. If requested, information/documents showing the damage you have suffered should be sent to our company.
